POSITION: • Candidate could be board-certified in hospice and palliative medicine, fellowship trained in palliative care/hospice medicine or a board-certified family medicine, internal medicine, med/ped trained physician with willingness to obtain hospice medical director certification. • This position is 50% clinical practice and 50% physician administration. Opportunities for research available. • This physician will participate in a 1:4 call schedule. • Those with academic interests could enjoy participation in new palliative care medicine fellowship program with the University of Illinois College of Medicine at Peoria. This is not mandatory. • Physician will drive coordination of care across the continuum for patients with serious illness by execution of the home care & post-acute services division clinical strategy. • Leads palliative care services (inpatient & outpatient), hospice (which includes a 16-bed free-standing hospice home), advance care planning, house calls program, and SNF practice. • Works with other region home care/post-acute medical directors, home care and post-acute VP of Clinical Services and VP Clinical Operations/CNO to ensure all work of the division supports overall medical group and ministry focus on high quality person centric care in a value based population health environment. • Provides expertise as a Subject Matter Expert (SME). The Region Medical Director will lend their role as a SME to support knowledge, expertise and leadership for the division and Ministry wide initiatives/services.
