Two Rivers Behavioral Health System has been providing services to the Kansas City metropolitan and surrounding area since 1986. Two Rivers is committed to providing children, adolescents and adults with compassionate behavioral health treatment based on the unique needs of the individual. We are a Joint Commission Accredited hospital providing inpatient, partial hospitalization and intensive outpatient services.
The LPN is responsible for assisting in the observations, planning, interventions and evaluation of care to the patients within the area assignment pursuant to the policies and procedures of Two Rivers Behavioral Health System with the supervision of an RN. Requires knowledge of nursing practices. The employee is required to be knowledgeable of and follow the Nurse Practice Act. Required to display a professional attitude in all aspects of his/her performance. Oriented and educated in gender, cultural differences and in age appropriate care for the assigned unit.
Demonstrates competent patient care based on their knowledge of growth & development for all age groups
Demonstrates competent patient care based on their knowledge of psychiatric and Chemical Dependency disorders
Assists in data collection, planning and implementation and evaluating of patients within scope of practice.
Observe and detects changes in patient(s) and takes responsibility to relay information to the registered nurse for on-going patient care.
Processes physician orders in a timely and accurate manner.
Able to provide patient/family education and administer medications in an accurate manner consistent with policy and good nursing practice.
Provides care based on knowledge of patients medical and nursing diagnosis.
Ability to prioritize and intervene with patients individually and in a group.
Documents patient status and care provided in a thorough and timely manner.
Responds appropriately to overhead Codes; e.g. code white, code blue etc.
Supervises assigned patients according to policy and procedure
Facilitates/leads educational and activity groups effectively
Completes admission and discharge processes, within the LPNs scope of practice in a thorough andtimely manner
Monitors the milieu for potential safety hazards and reports issues appropriately
Specific to Night Shift Staff: Audits the medical records to monitor timeliness and thoroughness of the medical record documentation in accordance with regulatory standards
Performs various other duties as assigned or as requested
