Advanced Practice Registered Nurses / Physician Assistants
The Lead Nurse Practitioner will spearhead Livio’s clinical mission—to provide exceptional palliative care to medically complex patients with limited life expectancy. In collaboration with our Executive Medical Director, the Lead Nurse Practitioner will design and build a palliative care model that enables patients to live with dignity and comfort while confronting serious health challenges. The Lead Nurse Practitioner will play a key role in training and mentoring Stella’s clinical team, including RNs, NPs and associated interdisciplinary teams, and will provide direct care to patients in their homes. Areas of Responsibility
Collaborate with Stella’s medical leadership to design patient assessments and clinical workflows that enable best-in-class care planning, care coordination and symptom control for Livio’s patients.
Design and implement a training curriculum for Stella’s staff of nurses and providers, who will provide in-home palliative care services to patients with advanced disease.
Supervise providers and nursing staff involved in provision of home-based palliative care.
Participate as facilitator/teacher in Stella’s ongoing internal clinical education.
Set measurable goals and objectives for Stella’s patient-care programs and clinicians.
Develop outcome measures and data streams that support evaluation of quality of care and continuous quality improvement.
Work collaboratively with a variety of Livio’s business and patient experience leads, to ensure strategic alignment between business and clinical workflows.
Provide in-home palliative care to patients age 65 and over.
Work collaboratively with community primary care physicians and specialists to craft individualized care plans for patients with palliative care needs.
Work collaboratively with area hospice providers and agencies to provide smooth transitions to hospice care for qualifying patients.
Lead ongoing quality improvement efforts as Stella refines its model of care.
Use telemedicine resources to provide clinical care, as appropriate.
Participate in Livio’s efforts to recruit exceptional clinicians.
5 years palliative care and hospice experience (preferred in outpatient and home-based settings)
3 years clinical leadership experience, including supervision and mentoring of other clinicians
Graduate of a school of nursing and accredited Nurse Practitioner training program
Valid Minnesota APRN license
Experience leading multidisciplinary healthcare teams
Ability to provide culturally-sensitive medical care in non-traditional settings, including patient homes.
Valid U.S. driver’s license.

Ability to use and manage multiple healthcare information technology platforms.
Ability to work collaboratively and independently, in settings without direct real time supervision.
Ability to adopt novel methods for patient care and coordination, and to provide feedback to Stella leadership on best practices.
