RECRUITMENT/RELOCATION INCENTIVE MAY BE AUTHORIZED This is an open continuous announcement. Education Debt Reduction Program: This position is eligible for the Education Debt Reduction Program (EDRP), a student loan reimbursement program within VHA. If selected, please contact the facility EDRP Coordinator. The Women's PACT Physician functions as the physician leader within a multidisciplinary PACT team assigned to provide comprehensive, collaborative acute and continuity care to an assigned panel of Veterans as well as to patients outside of the assigned panel when Veteran care indicates a need for such. Medical care is delivered in accordance with current clinical practice guidelines with attention to prevention via utilization of evidence-based clinical decision-making tools. In addition to the provision of high-quality medical care, PACT Physicians demonstrate excellence in customer service via compassionate and informed interpersonal relationships with Veterans, family members and staff. PACT Physicians report directly to the assigned Chief Medical Officer (CMO) and ultimately to the Associate Chief of Staff for Primary Care (ACOS/PC). As the Women's Health Clinical Champion (WHCC) are expected to meet the following performance standards including, but not limited to: - Ensures that routine women's health care services such as but not limited to annual pap smears, conception counselling, contraception counselling, management of menopausal symptoms & breast cancer screening are completed for women Veterans in accordance with VHA directive 1330.01 appendix B. - May serve as the Women's Health Medical Director within VATVCBHCS and perform duties in accordance with VHA directive 1330.01 appendix A. - Collaborates with the Women Veteran Program Manager to promote & support Women's Health Care at VATVCBHCS. - Third next available appointment within 14 days - A minimum of 40% or more encounters as Video Connect or Telephone visits - Encounter completion with-in 24hrs of patient appointment visit - Access to and demonstrated use of the Primary Care Almanac to identify high risk patients for proactive intervention - Daily attendance at PACT team let huddle - Attends clinic and team meetings regularly - Completion of Message Manager messages within 48hrs of message creation - Completion of Secure messaging response within 72hrs of message receipt - Maintain unprocessed view alerts at no more than 200 on any given day and no unprocessed Critical view alerts at the end of each day. - Ensure that all clinical reminders pertaining to HEDIS measures are completed at the time of the visit. (diabetes, prevention, ischemic heart disease, tobacco, depression/Alcohol/PTSD screening) - Consistently demonstrates high quality collegial interactions with all staff across VATVCBHCS. - Duties assigned by Chief Medical Officer Work Schedule: Monday - Friday, 8:00 A.M. - 4:30 P.M., with an occasional Saturday clinic or a week day extended hour clinic; however, subject to change, depending on the needs of the service/medical center or as agreed upon with the possibility of extended hours and weekends. NOTE: Candidates must be willing to work weekends, irregular work hours and extended hours. The candidate must be willing to be assigned to other related services and/or locations, if necessary.
