As an Nurse Practitioner - Urgent Care Center (UCC), you will provide safe and therapeutic care in holistic and systemic ways to include the assessment and overall management of care for a specified patient population under the supervision of a Medical Doctor. To help achieve our mission, you will exercise independent judgement in the assessment, diagnosis and initiation of medical processes and procedures in accordance with the Nurse Practitioner licensure regulations of the Commonwealth of Virginia and as delineated in the written Scope of Practice/Protocol for Nurse Practitioners. Demonstrating teamwork by communicating effectively and working collaboratively with all members of the healthcare team while displaying respect for and an appreciation of the contributions from all team members in order to foster positive working relationships is of vital importance. Utilizing evidenced based standards, guidelines and protocols for care delivery while incorporating data and information to continuously improve care and practice is required. Participating in quality/best practice initiatives by utilizing standards and guidelines for cost effective care delivery, in order to continuously improve patient care and enhance outcomes, is expected.
Job Responsibilities
- Recognizes behaviors that impact the team function and implements strategies to optimize teamwork while serving as a mentor for team members and facilitating the understanding of the role of the Nurse Practitioner for other healthcare providers.
- Demonstrates effective communication skills and assists in the resolution of conflict among healthcare team members, patients and families.
- Collaborates with the interdisciplinary team and incorporates their expertise to achieve patient outcomes.
- Participates in and supports departments, hospital and system-wide quality performance improvement initiatives. Provides accurate record keeping as evidenced by complete and pertinent history, physicals and progress notes.
- Reviews patients' plans of care with interdisciplinary team, the patients and their families. Evaluates/documents patients' responses to care and evaluates the impact of illness on patients' health status, functional status, nutritional status, sleep/rest patterns and family/social relationships while establishing a health promotion strategy.
- Performs and documents comprehensive physical and psychological exams of patients to identify priority care needs. Obtains and documents a detailed history, identifies patients at physiologic and/or psychological risk and prioritizes data based on immediate condition/needs. Practices in accordance with safe and effective standards of care and ensures compliance with professional and ethical standards of practice.
- Facilitates communication among departments to ensure continuity of care and establishes collegial relationships with physicians and other healthcare providers.
- Orders appropriate tests, treatments, medications and consults. Interprets and synthesizes laboratory, imaging and other pertinent data.
Additional Requirements
BLS; ACLS for critical care.
