We are part of St. Peter's Health Partners and are privileged to work with every hospital in the region to provide hospice services. We also collaborate with 43 skilled nursing facilities and many assistive living/adult homes across our service area. And the majority of our patients are served where they prefer to be at end of life to maximize quality of life - their home. We serve over 4,000 patients annually, with an average daily census of 540 patients. Our service area covers: Albany, Schenectady, Rensselaer, Saratoga, Columbia, Greene, Washington, and Montgomery Counties.
Physicians meet routinely with the entire hospice interdisciplinary group (nurses, social workers, chaplains) for care planning to meet each patient's and family's specific needs, serves as a resource to and collaborates with the hospice patient's community medical providers (including prescribing medications), is responsible for certifying the hospice patient's initial and ongoing eligibility for hospice services, helps to determine the appropriate level of hospice care, and may serve as the hospice patients' attending physician.
Our physicians visit patients in their homes, nursing homes, and hospitals as necessary, and routinely participate in the education of medical students, NP and PA students, as well as residents and Geriatrics Fellows. On-call responsibilities will be shared by a group of five Hospice physicians.
The Community Hospice is an innovative organization, directing a well-established Palliative Care Partners service line across our affiliated health system (inpatient, outpatient, and home-based). We provide a thorough orientation and supportive work environment.
