Advanced Practice Provider - Neuroscience

Position Summary:
Clinical Support for the CHS Neuroscience Service Line within the Catholic Health Physician Partners Medical Group
Responsibilities:
All phases of Neuroscience patient management including but not limited to:
- Medical management of the Neuroscience population in collaboration with other medical disciplines to ensure patient throughput across the CHS care continuum.
- Perform Neuroscience Consultations & perform clinical follow-up responsibilities as delegated by the established plan of care.
- Render patient specific care including ordering and reviewing laboratory & radiologic studies.
- Comprehensive clinical documentation in the EMR in compliance with Neuroscience coding initiatives.
- Educate patients, families & care providers with disease processes & implemented plan of care to ensure high levels of patient satisfaction.
- Coordination patient care plans with Consulting Physicians and other clinical providers involved in the management of Neuroscience patients.
- Maintain ongoing timely reports of clinical duties as required by CHS compliance initiatives.
Education, Training & Experience:
- Physician Assistant must be a graduate of a Physician Assistant program accredited by the Accreditation Review Commission on Education for the Physician Assistant. Physician Assistant's must have current NCCPA certification.
- Nurse Practitioner must be a graduate of an approved program for the training of NP's. Must have current and unrestricted Medical license. NP's must have current RN license and current NYS AANP or ANCC certification.
- Bachelor's Degree required. NYS licensed with certification.
- Previous Neuroscience/Surgical/Critical Care experience preferred.
- BLS and ACLS certification required
- Competency to practice medicine with physician supervision required.
- Basic computer skills including Microsoft Office and Outlook Email required.
- Familiarity with Electronic Health Records. (Epic Preferred)
