You take care of patients.
We'll take care of everything else.
You've spent years learning and training to provide outstanding care for your patients. That's why our model lets you focus on taking care of them. Our physicians have wrap-around practice and administrative support to do many of the things that aren't clinical, from answering emails to providing follow-up instructions, to helping secure social services for your patients.
We take care of you, too, with exceptional compensation packages, market-leading retirement contributions, gold-star health benefits, generous PTO, sabbatical opportunities, continuing medical education time and allowance, and leadership development through our in-house Northwest Permanente University.
Our integrated model fosters an environment that encourages and rewards innovation, providing our staff with ample opportunities to explore their interests. A recipient Healthgrades 50 Best Hospitals awards, Vohs and Lawrence awards, and 3-star ratings from Society of Thoracic Surgeons (to name just a few), we put innovative ideas forward while doing the right thing for patients. And we want your voice at our table.
Working for Northwest Permanente, you'll enjoy:
- 21% employer contribution to retirement programs, including pension

- Up to 95% employer-paid, best-in-class health plans for employee and dependents
- Student loan assistance program
- Relocation allowance
- Sign-on bonus
- Leadership opportunities
- Paid annual education leave + allowance

Required Qualifications/Position Description:
Northwest Permanente is seeking a Board Certified or Eligible Internist, Family Practitioner, Geriatrician or Hospitalist to join our growing medical group as part of our dedicated team of SNF Physicians in the Continuum Complexivist department.
In this role, you will work closely with an experienced, cohesive interdisciplinary team in our core Skilled Nursing facilities (SNF), and virtually, in our Medicare Home Health Physician program with a home base in Portland, OR. This team includes nurses, therapists and social workers that provide after-hospital care in the SNF setting. We emphasize transitions of care from the hospital to the skilled nursing setting and from a skilled nursing facility to home or other settings of custodial care. We provide a team approach, usually with one to two SNF physicians (MD/NP) and a Kaiser Care Coordinator. You will care for a spectrum of medical complexities, ranging from straight-forward, elective orthopedic surgery patients to those with multiple, complex medical problems at each facility.
Our department goals include decreasing emergency room visits and re-hospitalization, as well as transitioning to palliative and hospice care, when appropriate. You will enjoy a predominantly weekday, daytime work schedule. You will work most of the time at one of our 10 SNF locations, but also support a virtual care model that includes early, day, and evening shifts, once per week.
When you work virtual shifts, you will support our home health field staff, in members' homes, as well as consults (physician consults to determine patient eligibility). You will also support our "Transition Nurse Team" for escalations of patients transitioning from recent Hospitalist discharges or SNF discharges. This is roughly 10-20 percent of the work. You should have experience with digital technologies (AI platforms, telehealth and/ or digital health as examples). If you don't have direct experience, then you will need to demonstrate both flexibility and adaptability to incorporate these into your practice as we continue to evolve our virtual care model. We also feature a robust, Epic Health Connect-based charting.
