Position Summary
We have an immediate opening for a compassionate and skilled physician to join our Palliative Care Program, with a primary focus on building and expanding a robust community?based model of care. This role includes providing consultation services to patients and families, supporting symptom management, and helping guide complex medical decision?making. The position also includes participation in a shared rotation providing palliative care consults at a partnering local community hospital.

The Palliative Care Physician serves as a key medical resource to the interdisciplinary team, ensuring the highest standards of quality, communication, and continuity of care. This individual will support system-wide palliative care initiatives, champion best practices, and contribute to the development of innovative program strategies that improve patient outcomes and enhance quality of life.

The physician will supervise the nurse practitioner assigned to the program and is responsible for oversight of clinical practice, professional development, and collaboration between disciplines. In addition, the physician provides direct consultations, follow?up visits, and ongoing management for patients receiving community-based palliative care services.
